Case Management • Care Navigation • Advocacy Support

Hope for the City is a community-based care support service designed to help youth, Foster kids, individuals and families move from crisis to stability through resource navigation, benefits support, and ongoing case management oversight. Our mission is to restore hope, dignity, and direction for those who feel lost inside systems that are difficult to understand and even harder to navigate alone.

Our mission

Hope for the City exists to restore stability, dignity, and direction to kids, foster youth and young adults transitioning into independence. We provide guidance, advocacy, and resource navigation so that no young person ages out of care without support, structure, and someone in their corner. Our mission is to equip youth with the tools, connections, and confidence they need to build a future filled with possibility, purpose, and hope.

Our vision

Our vision is to become a citywide and nationwide support network that empowers foster youth and aging-out young adults to thrive emotionally, financially, academically, and socially. We envision a world where every young adult leaving care has access to safe housing, mental health resources, life skills, mentorship, and a community that believes in them — not just during transition, but throughout their journey into adulthood.

Our team

Hope for the City exists because one truth remains — no young person should age out of the system alone. Our team shows up for youth with consistency, compassion, and real-life navigation skills. We are listeners, advocates, connectors, and encouragers. We sit across from young adults not as officials, but as humans who see their potential.

Our work reflects guidance, accountability, life-skill readiness, emotional support, and resource navigation. Whether we are helping with housing, SSI forms, school transitions, or simply showing up when life feels heavy — we remain present. The youth we serve become more than cases to manage — they become young lives to walk beside.

We are here — not to replace family, but to be a dependable piece of stability where family may be missing.
